Rules of the Regents
Pursuant to sections 101, 207, 215, 216 and 217 of the Education Law
Effective March 9, 2006, the Board of Regents amended Regents Rules §3.27 pertaining to chartering and registration of museums and historical societies with collections, and §3.30 pertaining to incorporation and registration of historical societies without collections and cultural agencies.

In addition to the submission of the required New York State Board of Regents petitions and fees, the New York State Museum Chartering Office requires all organizations to fill out one of the following questionnaires. Please make sure that all questions receive an answer and when it deemed necessary provide additional information. The questionnaires with supporting documentation can either be mailed with the corresponding petition to New York State Education Office of Counsel, or separately to the Chartering Office by mail or email.

Note: A questionnaire is not required for an extension of the provisional charter or the amendment of any charters or certificates.
